Autism Evaluations

Active wait list - currently initial appointments are approximately 6 months out
Referrals from you primary care provider can be faxed to 856-212-1355
Evaluation can take up to 4-5 visits depending on complexity. Each visit is approximately 1 hour long.
You will be requested to bring IEP-504P evaluations, medical records, therapy evaluations, neurological work ups, as well as any psychological testing completed
The evaluations consist of observation, interviewing and use of multiple assessments.

Please find the age bracket your child is in currently and down load and complete all forms.

Complete the Developmental History form (See Above)
Each parent to complete a parent packet individually. If there is not a second parent a grandparent, close family friend, or relative can completed the 2nd packet
Teacher packet to be given to current daycare provider
